Hybrid Hospitalist/Urgent Care/Outpatient Family Practice MD
Board Certified Family Medicine or Internal Medicine
physicians for a hybrid position in Polacca, Arizona to cover Hospitalist, Urgent Care and Same Day Clinic Care at an IHS tribal facility. Ideal candidates are those who are available at least 7-days consecutively per month on a long-term basis. 2 weeks per month would be better.

Practice Description:

  • 10-hour shifts for 7 days consecutively ideal
  • ED covers nights
  • Schedule is Wednesday through Tuesday
  • Urgent care and Same Day coverage after rounds
  • Family-type admissions to ICU: febrile UTI, simple pneumonia, simple pancreatitis, end of life. No specialty care.

Details:

  • 4-bed inpatient unit, average is ~2 patients per day
  • Urgent Care: ~ 15-20 patients/day, though this is highly variable due to patient volume and acuity
  • Same day clinic: Typically scheduled in half-day blocks and each half day can have ~ 8-10 appointments/day. There is a variability in schedule and show rates.
  • Back up to RN care coordinators & PHN for unassigned patients
  • EHR: Resource and Patient Managment System (RPMS)
  • Nearest Airport Flagstaff Pulliam Regional (2 hrs), Phoenix (4 hrs)
  • Major trauma, acute surgical emergencies and high-risk obstetric patients are transported to Tuba City Indian Medical Hospital, Chinle Hospital, Phoenix Indian Medical Center or to several other contracted health facilities. Pediatric Admissions, referred out

Requirements:

  • Recent outpatient medical experience
  • Board certified in Family Practice or Internal Medicine.
  • Active, unrestricted license and DEA from any state or US territory
  • BLS, ACLS and (PALS optional)
  • Tuberculosis: Annual PPD or Chest X-ray, if there is a history of positive skin test.
  • Documentation of two (2) MMR (Measles, Mumps & Rubella) vaccinations or proof of immunity. (Employees born before 1957 do not need MMR or proof of immunity)
  • Documentation of two (2) varicella (chicken pox) vaccinations or proof of immunity or history of having disease.
  • Documentation of Hepatitis B series vaccination and influenza vaccination during flu season.
  • Documentation of COVID vaccination
  • Not had a lapse in practice > 6 months leading up to assignment

  • The service unit is located 150 miles from the Grand Canyon National Park, with excellent opportunities for photography and outdoor recreation. The facility has about 13,500 annual patient visits and offers 24 hours of physician coverage.
  • The Hopi people have lived in this area for more than a thousand years. Their village of Old Oraibi, is the oldest continuously inhabited community in the United States. While deeply bound to their rich heritage, most Hopi tribal members have adopted contemporary lifestyles and are bilingual. The Hopi Reservation is surrounded by the much larger Navajo Reservation. The past mingles with the present in the style of dress and the daily practices of many traditional Navajos. Often, the older patients do not speak English and interpreters are necessary .On both reservations, traditional healing continues to be a respected part of the culture. These and other traditions make providing health care at the Hopi Health Care Center a uniquely challenging and rewarding experience
