Job Responsibilities
Collaborate with cross-trained care management teams to provide integrated, coordinated care that improves patient outcomes.
Utilize data-driven tools and clinical insights to guide treatment plans and personalized care delivery.
Partner with highly trained support staff to streamline workflows and reduce administrative burdens.
Provide care virtually as needed, leveraging advanced telehealth platforms and optimized EMR technology.
Participate in quality improvement initiatives and population health programs to enhance care delivery and patient outcomes.
Maintain accurate documentation in the EMR system to ensure high-quality, compliant care.
Qualifications
2+ years of clinical outpatient experience post-residency preferred.
Active and unrestricted State medical license or eligibility for licensure in Arizona.
Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
Active DEA registration or eligibility to obtain one.
Current or eligible Medicare/Medicaid enrollment.
Skills
Proficiency with EMR systems; Athena EMR experience preferred.
Ability to work collaboratively in a team-based environment to deliver patient-centered, coordinated care.
Proficiency in utilizing clinical data and population health tools to drive outcomes.
