We are seeking Board-Certified/Board Eligible Family Medicine or Internal Medicine Physicians to join a forward-thinking, patient-centered practice located in the Memorial Village area of Houston, TX. Job Responsibilities

Deliver comprehensive, patient-centered primary care services in an outpatient setting.
Collaborate with cross-trained care management teams to provide integrated, coordinated care that improves patient outcomes.
Utilize data-driven tools and clinical insights to guide treatment plans and personalized care delivery.
Partner with highly trained support staff to streamline workflows and reduce administrative burdens.
Provide care via telehealth as needed, leveraging state-of-the-art EMR technology.
Participate in quality improvement initiatives and population health strategies to enhance overall care delivery.
Maintain accurate documentation in the EMR system to ensure high-quality, compliant care.

Qualifications

Board Certified or Board Eligible in Family Medicine or Internal Medicine.
2+ years of clinical outpatient experience post-residency preferred.
Active and unrestricted State medical license or eligibility for licensure.
Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
Active DEA registration or eligibility to obtain one.
Current or eligible Medicare/Medicaid enrollment.

Skills

Strong interpersonal and communication skills to build trust-based relationships with patients and care teams.
Comfortable with EMR systems; Athena EMR experience preferred.
Ability to work collaboratively in a team-based environment to deliver high-quality, coordinated care.
Proficiency in utilizing population health tools and clinical data to drive outcomes.
Commitment to patient-centered care and delivering an exceptional healthcare experience.
