Requirements:

• Board Certification FMOB – Required
• Active KS License – Highly Preferred
• Active BLS – Required for credentialing
• Active DEA – Required for credentialing
• Clean Malpractice – HIGHLY PREFERRED
• Procedure Logs – Required at presentation
