OSF is excited to launch an innovative new digital first primary care model, designed with the patient at the forefront of all we do. This service is focused on caring for patients 55+ with two or more chronic conditions, meeting them where they are in their healthcare journey. Creating efficiencies and more intentional time with our patients. Accessing healthcare can be challenging, but this care model is designed to meet those needs through virtual, in-person and group visits, offering options that work for them! This model will offer 24/7 digital care tools increasing access and human connection to deliver a more personalized patient care experience. With remote patient monitoring services, patients will be able to receive care outside normal business hours. Closing care gaps is critically important and will be managed through team huddles and care conferences to ensure interdisciplinary collaboration and intentional care coordination driving better outcomes for those we serve.
JOB SPECIFICS:
- OSF HealthCare is seeking a Primary Care Physician to lead the opening of a new model of care that will bring specialized primary care services to Evergreen Park and the surrounding communities.
- Strong clinical abilities for management of a population age 55+ with multiple chronic health conditions.
- Patient care settings will include clinic appointments, virtual visits, group visits, taking care to their home and hybrid options.
- Supports creating a culture of compassion, innovation, growth, and encourages open discussion of ethical and clinical concerns and willingness to give and receive real time feedback.
- Actively participate in mentorship with APPs and care team.
- Collaborate with cross functional teams.
- Help guide the advancement of the model development.

Qualifications
REQUIRED QUALIFICATIONS:
- Education: Doctorate Licenses/Certifications: Physician Licensed to practice medicine in current state.
- Board certified or board eligible in the appropriate specialty(ies). Obtain and maintain medical staff membership and appropriate privileges.
- Must be eligible as a provider for Medicare, Medicaid, and other federal health programs. Physician shall meet all qualifications to participate in OSF's programs for professional malpractice and other liability coverage.
- Licensed by the Federal Drug Enforcement Administration and the State of Illinois to prescribe narcotics, if applicable.

- Demonstrates the knowledge and skills necessary to provide patient care aligned to the preferences, goals, and needs of the patients served
