Job Title: Outpatient Family Medicine Physician

Job Summary:

We are seeking a dedicated and compassionate Outpatient Family Medicine Physician to join our healthcare team. In this role, you will provide comprehensive primary care to patients of all ages, from pediatrics to geriatrics, with a focus on preventive care, chronic disease management, and overall wellness. The ideal candidate will have a strong commitment to patient-centered care, outstanding communication skills, and the ability to collaborate effectively with a multidisciplinary team to improve patient outcomes.

Key Responsibilities:

  1. Patient Evaluation and Care

    • Perform comprehensive patient assessments, including medical history reviews and physical examinations.
    • Diagnose and treat a broad range of acute and chronic illnesses, injuries, and diseases across all age groups.
    • Develop, implement, and monitor individualized treatment plans that promote health and wellness and manage chronic conditions.
  2. Preventive Care and Health Education

    • Educate patients and their families on preventive health measures, including vaccinations, screenings, and lifestyle modifications.
    • Conduct routine health screenings and wellness exams, identifying risk factors and guiding patients on preventive care measures.
    • Provide counseling on diet, exercise, mental health, and other aspects of a healthy lifestyle to reduce the risk of future illnesses.
  3. Chronic Disease Management

    • Manage and monitor chronic conditions, such as diabetes, hypertension, asthma, and heart disease, with an emphasis on proactive care.
    • Coordinate with specialists and other healthcare providers to optimize care for patients with complex, multi-systemic conditions.
    • Develop individualized chronic disease management plans and ensure patient adherence through follow-up visits and check-ins.
  4. Collaboration and Teamwork

    • Collaborate with nurses, medical assistants, specialists, and other staff members to ensure seamless patient care.
    • Engage in team-based care initiatives to enhance the patient experience and improve care delivery.
    • Provide supervision and support to mid-level providers as needed, fostering a collaborative and educational work environment.

Qualifications:

  • Education: Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
  • Board Certification: Board-certified or board-eligible in Family Medicine.
  • Licensure: Active and unrestricted state medical license and DEA certification.
  • Experience: Prior experience in outpatient primary care or family medicine is preferred, but recent graduates are encouraged to apply.
  • Skills:
    • Strong diagnostic skills and proficiency in managing a wide array of medical conditions.
    • Excellent communication skills to effectively interact with patients, families, and the healthcare team.
    • Ability to prioritize patient needs, multitask, and work efficiently in a fast-paced clinic setting.
