Nurse Practitioner - Health Risk Assessment Program
Located in Hartford County, CT
Full-time, employed position
Position Details:
- Facility is seeking a dedicated Nurse Practitioner to join the Health Risk Assessment Program. This program is designed to support proactive and comprehensive care management of high-risk patients within primary care.
- Schedule: Monday through Friday, 9am to 5pm, with occasional Saturdays and virtual on-call duties.
- Participate in team meetings and case conferences.
- Provide supervision as needed.
- Conduct independent clinical and co-visits in field-based settings.
Requirements:
- Board certification or board eligibility (within one year of employment).
- AANC or AANP certification for Nurse Practitioners.
- Minimum of 2 years experience in an NP role.
- Experience working within an interdisciplinary team.
- Proficiency in HCC coding.
- Previous Health Risk Assessment experience.
- Preferred experience in telehealth and primary care provision.
