- 100%, office-based, outpatient Primary Care
- Seeing patients age 18 and up
- Schedule: Full-time, weekdays only (4-day work week options)
- Call: Infrequent phone call for work/life balance
- Access to Behavioral Health, High Risk Nurse Care Managers & Social Workers
