Job Responsibilities

Our Wound Care Physicians provide specialized wound care services to residents within skilled nursing facilities. This role involves evaluating, diagnosing, and managing acute and chronic wounds, including but not limited to pressure ulcers, venous and arterial ulcers, diabetic foot ulcers, and traumatic wounds. The physician will work in a collaborative environment with facility staff to create and implement individualized wound care treatment plans aimed at promoting wound healing, preventing complications, and improving patient outcomes.

Primary Responsibilities:

Patient Assessment and Diagnosis: Conduct comprehensive wound assessments, including physical examinations, imaging, and lab work as needed, to accurately diagnose wound etiology and stage.

Treatment Planning: Develop and implement evidence-based treatment plans, incorporating advanced wound care modalities such as debridement, use of wound dressings, and topical or systemic therapies. Recommend vascular consultations and vascular testing as clinically indicated.

Procedures and Treatment: Perform necessary wound care procedures, including sharp and enzymatic debridement, application of specialized dressings, and management of any wound-related infections. Ensure all procedures align with current clinical guidelines and standards.

Patient and Family Education: Educate residents and their families on wound care management, prevention strategies, and the importance of adherence to treatment plans. Provide clear guidance on wound care practices that can be performed by facility staff and families when appropriate.

Collaboration and Communication: Work closely with facility nurses, CNAs, dietitians, and other interdisciplinary team members to monitor wound progress and adapt treatment plans. Act as a resource for facility staff, offering guidance on wound care best practices.

Documentation and Compliance: Ensure accurate and timely documentation of wound assessments, treatment plans, procedures performed, and patient progress in the electronic medical record. Maintain compliance with federal, state, and facility regulations related to patient care and documentation standards.

Follow-up and Re-evaluation: See residents weekly for wound reassessment, modifying the treatment plan as needed based on wound progress until each wound is fully resolved.

Quality Assurance and Improvement: Participate in facility quality improvement initiatives and wound care program assessments. Collect and report data related to wound healing rates, infection rates, and other relevant quality indicators.

Professional Development: Stay current with advancements in wound care treatments, technologies, and best practices to ensure optimal patient outcomes and maintain compliance with state licensure and certification requirements.

This role requires a high level of clinical expertise, compassionate patient interaction, and strong collaboration skills, with a focus on enhancing quality of life for residents and supporting overall facility objectives in wound management.

Qualifications

Current NY and/or NJ Medical License
Reliable Transportation
CDS, DEA, ACLS, BLS
Wound Care Certified or willing to become Wound Care Certified
Three plus years of experience as a family practice physician, including experience doing minor procedures
Exceptional communication and customer service skills
Self motivated and able to work independently

Working Hours

Monday through Friday
Hours are based on assigned facilities , however rounds typically start at 7 am.
You will cover call for your assigned facilities, however this is minimal.
Weekends are only for emergencies and are extremely rare.
