A community health center in south-central Vermont is adding a Family Medicine Physician due to retirement. Our mission is to service our patients with quality, efficiency, accessibility, and equity.


About the opportunity:

  • Total outpatient
  • Work-life balance with 4-day work weeks
  • On-Call is just three weeks per year on average
  • We are a multi-site, Federally Qualified Health Center serving 80% of the primary care needs in central Vermont
  • We are the largest FQHC in northern New England
  • Our sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place
  • We have a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners, and Licensed Clinical Social Workers embedded within its primary care sites
  • We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.
