Job Description
Providence Medical Group at Home is seeking a part-time physician to join a skilled interdisciplinary team dedicated to providing home-based urgent, consultative, and primary care to patients in a community setting. In this role, you will provide consultations for acute conditions, geriatric assessments, and transitions of care, in addition to limited primary care for a group of complex, home-bound patients who are not best served in a PMG clinic setting. - Part-time (0.6 FTE)
- Outpatient, primary care in a home-based, community setting
- Work within an interdisciplinary team including pharmacy, care management, field nursing, medical assistants, community paramedics and a chaplain
- Ideal candidates will have previous clinical experience working in primary care, home health, geriatrics, and/or palliative medicine
