Primary Care Physician - PACE Program (All-Inclusive Care for the Elderly)
Modesto, CA


Join our innovative PACE (Program of All-Inclusive Care for the Elderly) team, where we provide seniors with the support they need to continue living in their homes and communities. Our approach treats the whole person, not just the symptoms, through an interdisciplinary team (IDT) dedicated to participant care. As a Primary Care Physician, you will play a key role in delivering comprehensive medical services, ensuring quality care, and supporting both participants and staff.

Compensation & Benefits

  • Base Salary: USD $250,000 to $280,000

Qualifications:

  • Doctorate Degree (MD, DO)

  • Board Certification in Internal Medicine, Family Practice, Emergency Medicine, or Geriatrics (preferred)

  • Active California Physician License without restrictions

  • DEA Registration and State CDS Certification

  • Residency program completion accredited by the Accreditation Council for Graduate Medical Education

  • Experience with geriatric patients is preferred

Responsibilities:

  • Conduct in-person comprehensive assessments of participants and develop care plans in collaboration with the IDT.

  • Oversee care for participants in nursing homes and hospitals, including admits, discharges, and coordination with medical specialists.

  • Regularly visit participants in nursing homes to ensure continuity of care based on individual needs.

  • Participate in rotating night and weekend call schedules.

  • Collaborate with community-based physicians and the IDT on discharge planning for acute or long-term care settings.
