Responsibilities Include:
- Providing clinical care to acutely ill patients via virtual or in-person visits.
- 50% administrative and 50% clinical.
- Overseeing clinical documentation, treatment plans, medication management, and providing medical direction.
- Supervising and collaborating with Advanced Practice Providers (APPs).
- Driving the growth of the program while maintaining high standards of clinical quality and safety.
- Performing daily patient care visits and completing all necessary clinical documentation and orders.
- Working with a multidisciplinary team to identify patients suitable for the program and provide comprehensive care.
- Facilitating in-home diagnostics, point-of-care testing, and patient monitoring.
- Participating in quality improvement initiatives to advance the program's care model.
