We are seeking a board-certified Family Medicine physician with acute inpatient care experience to join a growing and innovative Hospital-at-Home program. This is an exciting leadership opportunity where patients receive hospital-level care within the comfort of their own homes.

Hospital-at-Home Physician Role

This position is an opportunity to join a growing team where you will oversee the clinical, quality, and safety components of inpatient care in both the home and virtual environments. Your role will be divided between delivering high-quality clinical care to acutely ill patients and leading the growth and development of the program. As the program expands, there will be continued opportunities for further leadership roles across the ministry.

Responsibilities Include:
  • Providing clinical care to acutely ill patients via virtual or in-person visits.
  • 50% administrative and 50% clinical.
  • Overseeing clinical documentation, treatment plans, medication management, and providing medical direction.
  • Supervising and collaborating with Advanced Practice Providers (APPs).
  • Driving the growth of the program while maintaining high standards of clinical quality and safety.
  • Performing daily patient care visits and completing all necessary clinical documentation and orders.
  • Working with a multidisciplinary team to identify patients suitable for the program and provide comprehensive care.
  • Facilitating in-home diagnostics, point-of-care testing, and patient monitoring.
  • Participating in quality improvement initiatives to advance the program's care model.
