Job Summary: The Primary Care Provider will play a vital role in delivering comprehensive and culturally sensitive
healthcare services to patients of our Health Clinic. The candidate must have a strong
commitment to improving health outcomes, and promoting wellness among our patients.
Duties/Responsibilities:
Provide primary care services including diagnosis, treatment and management of acute and chronic
conditions.
Perform routine examinations, preventative care and health education to patients of all ages.
Develop and implement individualized care plans in collaboration with patients and their families.
Coordinate with multidisciplinary team members including specialists, nurses and social workers to
ensure continuity of care.
Maintain accurate and timely patient records in accordance with clinic policies and procedures.
Participate in community outreach and health promotion activities to enhance public health
awareness.
Uphold patient confidentiality and respect cultural sensitivities in all interactions.
Engage in continuous professional development and training to stay current with medical advancements
and best practices.
Performs other duties as assigned.
Qualifications:
Medical degree (MD or DO) or Advanced Practice Provider (NP or PA) with a valid state license to practice
REQUIRED.
Board certification in Family Medicine, Internal Medicine or related specialty PREFERRED.
Experience working in a primary care setting; experience with Native American populations PREFERRED.
Strong interpersonal and communication skills.
Ability to work effectively in a team-oriented environment.
Commitment to providing culturally competent care.
Pre-employment/random drug tests and a background check will be required for this position.
