Requirements:

• Board Certified General Surgery – Required or Truly BE (0-5 years residency completion)
• Active CO license – Required or IMLC (LOQ in hand at time of submission)
• Residency Requirements/Fellowship in General Surgery – Required
• Active ATLS BLS – Required
• Candidate must be available for any of the shifts detailed in the job description – Required
• Candidates must detail all of the shifts/stints they are able to offer to the team through December – Required
• Active DEA – Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked)
• Clean Malpractice/Background – Required
• Must include a copy of IMLC, ATLS, BLS, DEA at time of name clear PLEASE REFER TO THE RULES OF ENGAGEMENT FOR NAME CLEAR AND PRESENTATION REQUIREMENTS!
