As we expand rapidly in 2024,
we are seeking compassionate, resourceful, and dedicated Community Care
Navigators to support patients and caregivers dealing with neurodegenerative
diseases. Ideal candidates will have experience in clinical care, geriatrics,
and/or working with patients with dementia. Bilingual fluency in Spanish, both
verbal and written, is essential to support our diverse patient and caregiver
population. This fully remote position provides the necessary technology to
work from home, with occasional travel to our headquarters in Chicago, IL, or

Compensation & Benefits: Position is full time/exempt with competitive salary and benefits package including health insurance offering. Salary range for this role is competitive depending on the candidate’s level of experience.
Requirements:
- High school diploma or equivalent, with sufficient experience and proven skills to perform assigned duties effectively.
- Proficient in using technology to support members remotely (telephone, text etiquette, virtual visit platforms, etc.).
- Excellent verbal and written communication skills, organizational abilities, and interpersonal skills to work effectively within a diverse team.
- Knowledge of scheduling platforms to ensure accurate appointment scheduling and management.
- Experience with electronic health record systems and/or care facilitation platforms for accurate documentation.
- Skilled in collecting and documenting member clinical and demographic data promptly.
- Strong problem-solving skills, with the ability to make difficult decisions and collaborate with team members when necessary.
- Capable of providing creative solutions to challenges within the healthcare system to optimize members’ care and health.
- Possess a growth and learning mentality, with the ability to think outside the box and take on non-traditional responsibilities.
- Adaptable to change, prepared for frequent, fast-paced changes, and shifting priorities.
- Ability to establish cooperative working relationships with patients, teammates, and healthcare and community service providers.
