JOB SUMMARY:

LPG Physicians are responsible for: (i) delivering age and specialty appropriate medical care through the process of assessment, evaluation, diagnosing, treatment and follow-up; (ii) performing responsibilities in four areas of practice clinician, teacher, leadership and professional development; (iii) collaborating with colleagues and other professional disciplines to ensure safe, effective, efficient and value added patient centered care and to achieve desired patient outcomes; (iv) continuously monitoring quality patient care through excellence in customer service and performance improvement that demonstrate positive outcomes in patient care; (v) providing leadership in the LPG practice location and throughout Lee Health; (vi) possessing excellent communication skills with patient, family, caregivers, multidisciplinary team members, medical staff members, and all employees; (vii) keeping quality and safety in the forefront in the delivery of patient care, and leading the care team in the recognition and prevention of adverse events; and (viii) participating/supporting the integrated continuum of care of patients within Lee Health/LPG.

The LPG Virtual Primary Care Physician will be a 100% remote role that will support the primary care initiatives for value based care and virtual care of our patient population. This role will include but not be limited to performance of wellness visits for attributed patients, virtual visits for immediate care or follow up of chronic disease management, and the supervision of virtual advanced providers who are assisting with in-basket support to the primary care division.

JOB REQUIREMENTS:

Education: LPG Physician must be a graduate from an accredited medical school and residency program.

State of Florida Licensure: LPG Physician must have and maintain an unrestricted, active license to practice medicine or osteopathic medicine, as applicable, in the State of Florida.

Board Certification: LPG Physician must be board certified or board eligible in their primary specialty and any subspecialty required by Lee Health. If board eligible, the LPG Physician must become board certified in such primary specialty and subspecialty, as applicable, within two (2) years of commencing employment with Lee Health or any other timelines as outlined within the terms of the agreement.

Other: LPG Physician must meet credentialing requirements as established by the Lee Health Medical Staff Services Department and must possess communication and interpersonal skills, basic computer skills and customer service skills.

JOB DUTIES: LPG Physician job duties include the following:

1. Effectively identifying and addressing disease prevention and health promotion opportunities, and advising appropriate follow up with their primary care physician.

2. Maintaining accurate medical records in accordance with applicable Lee Health Medical Staff and LPG policy. Documenting in a timely manner all medical evaluations, diagnoses, procedures, treatment, outcomes, referrals and consultations consistent with applicable National Committee for Quality Assurance (NCQA), DNV GL - Healthcare, Florida law and other accreditation and regulatory standards. Utilizing the Electronic Health Record (EHR) to its full capacity.

3. Documents encounters in a timely manner and has the encounter closed within 24 hours.

4. Reviews test results, responds to telephone/MyChart messages and refill requests in a timely manner and is consistent with LPG policies on Response Time Expectations.

5. Adhering to Lee Health Medical Staff bylaws, rules and regulations. Creating an atmosphere that promotes and maintains professional standards. Meeting and following LPG rules and regulations.

6. Supporting the philosophy, objectives and mission of Lee Health and LPG.

7. Treating all customers and team members with courtesy, dignity and respect and giving careful consideration to the views, opinions and feelings of others.

8. Maintaining a high level of patient satisfaction and demonstrating positive interpersonal skills in dealings with others.

9. Collaborating with advanced providers; embracing and supporting advanced providers as part of the clinical team; and providing direction and reviewing the work, records, and practice of the advanced provider on a continual basis to ensure that appropriate directions are given and understood, and that appropriate treatment is rendered to patients.

10. Using leadership skills to lead a team, lead change, embrace relationships; and working with advanced providers and all employees to the top of their license or job description.

11. Participating in departmental, organizational, and/or community committees/activities as appropriate.

12. Analyzing new knowledge gained from conferences, workshops, professional literature and assimilating this knowledge into clinical practice.

13. Supporting and practicing the Lee Health Physician Compact.

14. Contributing to medical knowledge by synthesizing new research and medical knowledge. Uses knowledge to improve healthcare and helps scale best practices.

15. Participating in and supports the integrated continuum of care of patients within Lee Health to enhance the quality of care.

16. Embracing a sense of community and a commitment to maintaining high standards of performance.

17. Cultivating and maintaining collegial and collaborative relationships with colleagues and staff.

18. Attending quarterly LPG provider meetings; monthly practice meetings; and such other meetings as requested by the Chief Medical Officer, Chief Officer, System Medical Director or Chief Physician Executive.

19. Embracing and supporting LPG programs and Lee Health programs.

20. Meeting quality metrics as may be set forth from time to time by Lee Health; and

21. Performing such other duties as assigned by the Chief Medical Officer, System Medical Director, or Chief Physician Executive.
