Requirements:

• Board Certified Family Medicine or Board Eligible FM – Required
• Active CA license – Required
• OB Fellowship – Required
• Active ACLS DEA PALS (for peds call) – Required
• Must state availability (# of shifts per month) at time of name clear – REQUIRED
• Must be qualified to handle c-sections – REQUIRED
• Clean malpractice/Background – REQUIRED
• Local no flights – Highly Preferred
• Disclose if provider has worked at any other CHI/Dignity Facility – REQUIRED
• COVID Vaccine – Required
• Must include copies of ACLS DEA PALS at time of name clear
