- Provider must have minimum of 3-5 years in practice in outpatient primary care and interested in leadership opportunities._x000D_
- Rural location. Population of 2138 in 2019 with 839 of them over age 55. Median age 52 years. VERY grateful patients!!!_x000D_
- Brand new practice in Colebrook. Lots of available volume with stable practice offering continuity of care._x000D_
- Seeking a provider who plans to stay and practice 5-10 years minimum._x000D_
- Would consider Board Certified Family Medicine or Geriatrician_x000D_
