**Locums General Dentistry Position in Fortuna, CA**
**Coverage**

**Requirements**
- Minimum Board Certification: Eligible
- License Requirement: Licensed in CA only
- The Following Certifications Are Required: BLS
- Estimated Credentialing Timeframe: 45-60 days
**Work Schedule**
- Monday-Friday, 8-5
- The provider must be willing to work a minimum of 3 months with the option to extend.
**Specialty Skills Required**
- General dentistry skills covering a patient mix of 50/50 kids and adults.
**Additional Information**
- No supervision of Advanced Practice Providers required.
- Active DEA needed before start.
- Chart closure within 72 hours.
- Credentialing requires specific immunizations including the 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
- Primary series plus booster, regardless of the date received OR 1 dose of the current COVID vaccine (2023/2024 formulation).
