Nurse Practitioner - Family Practice

ABOUT THE POSITION:

· Schedule generally, M-F 8am-5pm occasional Sat mornings, early morning or early evening meetings

· Responsible for obtaining complete medical histories, performing physical examinations, ordering appropriate diagnostic tests from patients

· This person will work in collaboration with physicians in providing diagnostic and treatment services to patients.

· Will assess, diagnose, and develop a plan of care in accordance with practice protocols

· Non-physician providers are valued members of the clinical practice with faculty physician supervision and active participation in a clinical teaching environment

· 20/40 minute durations with patients

Job Responsibilities

The candidate will work in a busy Family Medicine practice that delivers comprehensive patient care in an ambulatory outpatient setting

· Fast paced family practice with many opportunities for growth. Practice includes Physicians and Advanced practice providers

· Opportunities to practice with the Physician and/or with your own Independent panel of patients.

· State of the art facility with other specialty practices nearby to streamline services for patients. Radiology, laboratory, cardiac, sports, PT, OB, urology services available within building

· Epic is the EMR utilized in office

Qualifications

NYS licensed/registered NP (NYS NP specialty area of certification must be consistent with practice).

· Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire (eligibility criteria for NP requires National Certification through AANP or ANCC, and master’s degree). Initial certification must be present or obtained within 6 months of hire

· Current CPR certification through the American Heart Association or the American Red Cross is required

· Minimum 3-5 years previous NP experience in primary care outpatient is preferred
