Providing routine care for illness, injury and chronic conditions for patients of all ages, medical center is conveniently located in East Greenbush, just minutes from Exit 9 on I-90. The practice has seven physicians and five advanced practitioners and is well established-enjoying strong area growth and ample access to sub- specialty care._x000D_

Our expert and caring providers offer adult and geriatric health care; pediatric exams; routine immunizations and preventive inoculations; preventive health and wellness care; and patient education. The practice is NCQA certified as a level 3 Patient Centered Medical Home._x000D_
