Contract Length: 3 months minimum
Shift Details: Mon-Fri (either 4 tens or 5 eights)
Schedule: Full time
Work Details:
· Amount of Coverage Needed per Month: full time, Mon-Fri
· Patients per day: 15-20, outpatient
· Patient Population/Age: all ages, mainly adult
· Department Description: outpatient clinic
· EMR: Athena Practice
Practice Setting:
• Clinic/Private Practice- somewhat rural area
Requirements:
· Board Certification: yes, required
· License Required: AZ license
· DEA: yes
· Certifications Required: BLS
· Minimum Experience Required: 1-2 years family practice experience in outpatient setting
Credentialing Timeframe: 30-45 days
*Must adhere to vaccination policy
