**Position Details**

- Established practice serving the West Central Illinois community
- Current providers include two Physicians and one Advance Practice Provider
- Experienced staff, both clinical and non-clinical who know the patients and the community extremely well
- Dedicated administrative time, with flexible scheduling to ensure sufficient patient access
- Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination

**Responsibilities**
- Primary care for patients of all ages; average 20-25 patients per day
- Call schedule requirement: Provider takes second call if needed after initial MyNurse triage
- Excellent communication (written and verbal), analytical, and problem-solving skills
- Ability to work as a team member to improve the health of the people and communities we serve
- Provide the best outcome for every patient every time
- Hospital rounding in a local community hospital (newborn to adult) is an option, but not required
**Requirements**
- MD or DO
- Board Certified in Family Medicine
- Hold an unrestricted license to practice medicine and surgery in the State of Illinois
- Eligible to work in the United States
