May 23rd or 31st – 3 months with option to extend

Minimum Length of Coverage:

• 3 months preferred

Schedule:

• Monday-Thursday or Tuesday-Friday
• 4 x 10 hour Day Shifts

Call Schedule if Required:

• No Call

Type of Facility (Clinic, Hospital, Multi-Specialty Group, Solo, Community Health, etc.):

• Outpatient

Position Details:

• Outpatient coverage
• General Primary Care coverage
• Average Patient Volume: 2-2.5 per hour
• Must have an active Arizona License
• Must have current BLS certification
• COVID vaccination required

Inpatient and/or Outpatient:

• All outpatient

Support Staff Available:

• Full Clinic Support including other Primary Care providers and Pediatricians

Additional Details:

• Temporary privileges within 1-2 weeks
