Job Description:
- Conduct thorough evaluations and assessments of patients, including taking medical histories, performing physical examinations, and ordering diagnostic tests.
- Develop and implement individualized treatment plans based on patient needs, medical history, and diagnostic results.
- Provide preventive care services, including routine check-ups, immunizations, and health screenings.
- Manage chronic conditions such as diabetes, hypertension, and asthma, and provide ongoing monitoring and treatment.
- Address acute medical issues, including infections, injuries, and urgent care needs.
- Educate patients and their families about health maintenance, disease prevention, and lifestyle modifications.
Collaboration and Communication:
- Collaborate with other healthcare professionals, including specialists, nurses, and support staff, to provide coordinated and comprehensive care.
- Communicate effectively with patients and their families about diagnoses, treatment options, and care plans.
- Participate in care coordination and case management to ensure seamless patient care.
Clinical Management:
- Maintain accurate and up-to-date medical records in accordance with hospital/clinic policies and regulatory requirements.
- Participate in quality improvement initiatives and clinical audits to enhance patient care and operational efficiency.
- Stay current with medical advancements, best practices, and guidelines in primary care.
Qualifications:
- Education: Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
- Training: Completed an accredited residency in Internal Medicine, Family Medicine, or a related field.
- Board Certification: Board-certified or board-eligible in Internal Medicine, Family Medicine, or a related specialty.
- Experience: 2 years of experience in primary care preferred. Exceptional new graduates with outstanding training will also be considered.
- Skills: Strong diagnostic and clinical skills in primary care. Proficiency in utilizing electronic health records (EHR) systems.
