A leading healthcare organization in Albuquerque, NM is seeking an experienced Nurse Practitioner (NP) or Physician (MD) to join their hybrid primary care team. This role offers a unique opportunity to provide comprehensive, patient-centered care through a combination of in-person and telemedicine services.

About the Opportunity:

• Schedule: Flexible, 30-40 hours per week
• Setting: Primary Care
• Patient Population: All ages
• Call Schedule: None
• Hybrid Role: Combination of in-person care and telemedicine (asynchronous chat & synchronous video)

Responsibilities:

• Providing comprehensive, evidence-based primary care to patients
• Managing chronic and complex conditions, including diabetes, hypertension, obesity, and mental health disorders
• Utilizing proprietary digital tools and telehealth platforms to optimize patient care and engagement
• Collaborating with a multi-disciplinary team to develop and implement individualized treatment plans
• Ensuring culturally competent, compassionate care for a diverse patient population
• Efficiently addressing patient concerns while maintaining a team-oriented, problem-solving approach

Qualifications:

• Minimum 2 years of Clinical experience in a Primary Care setting
• Active New Mexico medical license (or ability to obtain one)
• Board Certification in Primary Care, Family Medicine, or Internal Medicine

Desired Skills:

• Proficiency with Digital Health tools and Telemedicine platforms
• Experience in multi-specialty Primary Care settings
• Background in Value-based Care models
