Primary Care Physician – Mobile, AL

Position Summary:
As a Primary Care Physician (PCP), you will take the lead in delivering patient-centered care within a team-based model. Your role will focus on evaluating, diagnosing, and treating patients while collaborating with a wide range of healthcare professionals, including advanced practice providers, pharmacists, care associates, and behavioral health support. You will work to provide high-quality, coordinated care for senior patients, ensuring they receive a concierge experience that addresses all their healthcare needs.
Key Responsibilities:
- Patient Care:
- Evaluate and treat center patients in accordance with the highest standards of care.
- Coordinate patient services, including specialty referrals, hospital and Skilled Nursing Facility (SNF) coordination, durable medical equipment, and home health services.
- Actively participate in daily care team huddles, contributing medical expertise and sharing patient insights.
- Engage with patients, fostering a trusting and supportive environment while addressing their needs comprehensively.
- Collaboration and Teamwork:
- Work closely with the Lead Physician and Center Administrator to foster a culture of cooperation and teamwork within the practice.
- Serve as a key source of medical expertise for the care team, guiding decision-making and best practices.
- Documentation & Reporting:
- Complete all medical record documentation on a daily basis in collaboration with a quality-based coder to ensure comprehensive and accurate patient information.
- Participate in quality reviews with the Lead Physician to ensure high standards of care and improve patient outcomes.
- Patient-Facing & Administrative Balance:
- Focus primarily on direct patient care, with additional time dedicated to charting, meetings, and other administrative tasks related to patient care.

Required Qualifications:
- Graduate of an accredited MD or DO program.
- Current and unrestricted medical license (or willingness to obtain a license in the state of practice). Eligibility to obtain licensure in other states as required.
- Board Certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
- Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
Additional:
- This position is patient-facing and requires a TB screening upon selection.
Preferred Qualifications:
- Knowledge of HEDIS, STARS, CAHPS, and HOS measures.
- Experience working in medically underserved and culturally diverse communities.
- Familiarity with electronic health records (EHR) systems.
- Experience working in Medicare Advantage Global Risk Models.
