Overview:
Foundation Medical Partners, an affiliate of Southern New Hampshire Health, is seeking a compassionate and skilled Geriatric Physician to join our team and provide specialized care across our five Post-Acute Care Unit (PACU) nursing home facilities located throughout the Southern New Hampshire area. This is an excellent opportunity for a physician who is dedicated to delivering high-quality care to elderly patients and improving their quality of life in a post-acute care setting.

Qualifications

Qualifications:
Board-certified or board-eligible in Geriatrics or Internal Medicine with a focus on elderly care
Experience in post-acute, long-term care, or geriatric settings preferred
Strong clinical and diagnostic skills with a focus on geriatric care
Excellent communication and interpersonal skills with a dedication to compassionate patient care
Ability to work effectively as part of an interdisciplinary team
NH Medical License (or eligibility to obtain)

Working Hours

8am-5pm call rotation
