Responsibilities:
• Provide primary care services to senior care clients at participating long-term care facilities
• Manage admissions, including initial history and physical examinations
• Handle discharges to homes, funeral homes, hospitals, or assisted living facilities
• Treat acute medical conditions
• Perform regulatory duties such as chronic care management and routine care visits
