We are seeking a dedicated Geriatric Medicine Physician to join our healthcare team in West Springfield, MA. This position primarily serves as an outpatient Primary Care Provider (PCP) for elderly patients and involves providing care in a skilled nursing facility. The ideal candidate will have a passion for geriatric care and a commitment to enhancing the quality of life for our frail and elderly population.

In this role, you will provide comprehensive outpatient primary care to geriatric patients, conducting thorough assessments and creating individualized care plans tailored to each patient's needs. You will collaborate with multidisciplinary teams to ensure optimal patient outcomes and participate in skilled facility care, providing guidance and support to nursing staff. Additionally, you will manage and address patient concerns via telephone triage during on-call shifts.

The position requires an average of seven (7) on-call days per month, including evenings and weekends, along with weekend telephone triage responsibilities as needed. You can expect to see an average of 2 to 8 patients per day, depending on the day’s schedule.

Compensation: $208,000 - $241,000 plus benefits including:

Qualifications:

· Board Certified in Internal Medicine or Family Medicine (Required).

· Board Certified in Geriatric Medicine (Preferred).

· Valid Massachusetts medical license or willingness to obtain (Required).

· CDS Certification & DEA Verbal Confirmation Registration (Required prior to start).

· Minimum of 1 year of experience working with frail or elderly populations (Required).

· Current BLS certification (Required prior to start).
