The Physician will provide direct primary medical care to patients and supervise medical care personnel, including PCP Nurse Practitioners, RNs, and Certified Medical Assistants. The Physician will deliver care within the Interdisciplinary Team framework.

  • Setting: Outpatient clinic

  • Population: Geriatrics


What you will be doing:

  • Conduct comprehensive history and physical assessments for new referrals.

  • Perform interval history and physical exams for established participants, conducted quarterly or as needed.

  • Evaluate and treat participants with episodic illnesses.

  • Make referrals to Medical Specialists as necessary.

  • Admit participants to the hospital when appropriate, providing primary care responsibilities during hospitalization and coordinating discharge planning with the IDT.

  • Oversee the care of participants residing in nursing homes, including regular visits and communication with nursing home staff.

  • Engage in family conferences for care planning and implementation.

  • Coordinate 24-hour care delivery.

  • Document participant changes accurately in medical records and communicates these changes to the IDT promptly.

  • Participate and, attend staff meetings, and contribute to participant care planning.

  • Stay updated on quality/clinical standards and best practices for the geriatric population.

  • Contribute to Ethics and Quality Improvement Committees or work groups as assigned.

Qualifications

What you must have:

Valid MD/DO license in good standing to practice medicine in the State of practicing.

Board Certification

Graduate of an accredited School of Medicine.

Minimum of two years of clinical experience required.

Experience working with a frail or elderly population preferred, or willingness to undergo appropriate training upon hiring.

Knowledge of geriatric medicine and the special needs of complex geriatric patients.

Fellowship or additional training in Geriatrics preferred.

Understanding of the culture and needs of socially and ethnically diverse populations
