You will participate as a member of the PACE Interdisciplinary Team in the care of high-risk older adults living in the community with a strong focus on the participant's goals as well as excellent geriatric care management. QUALIFICATIONS - At least one year of experience working with a geriatric population - Current MA license, DEA prescription privileges, and CPR Certification Preferred - BC in Geriatric Medicine - Suboxone Waiver - At least one year of experience working with low-income and diverse cultural groups Ability to - Quickly recognize situations/problems before becoming acute, implement effective solutions, and understand consequences - Convey information clearly and succinctly to team members
