Position Overview:
The Lead Primary Care Physician (Lead PCP) is a key role in our clinical leadership track. You will be responsible for both patient care and clinical leadership within our center. This position offers an opportunity to make a significant impact on patient outcomes while guiding and developing your care team.
Key Responsibilities:
- Patient Care: Independently manage a panel of at least 360 patients, providing care for acute and chronic illnesses, and ensuring high-quality outcomes.
- Leadership: Lead weekly Super Huddle (SH) and Transforming Care Meetings (TCM), support center management, and fill in as needed for center leadership.
- Coaching: Act as a health coach, helping patients set and achieve health goals, and drive a positive, cohesive team culture.
- Team Management: Supervise and mentor primary care providers (PCPs), engage with transitional care teams, and collaborate with other healthcare professionals.
- Clinical Excellence: Oversee patient care coordination, participate in risk and quality management programs, and strive for continuous improvement in clinical practices.

Qualifications:
- Education: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Licensure: Current, active MD licensure in the state of employment.
- Experience: Minimum 1 year of clinical experience preferred; prior experience as a ChenMed PCP Partner is ideal.
- Certification: Board eligibility required; board certification preferred. DEA number and Basic Life Support (BLS) certification required.
