Jefferson Health and Thomas Jefferson University seek an experienced Geriatric Medicine leader to develop, expand, innovate, and coordinate geriatric clinical programs across Jefferson Health, and to create and execute on a visionary strategy for clinical geriatrics across Jefferson Health.
The successful candidate will join the faculty of the Division of Geriatric Medicine within the Department of Family and Community Medicine at Sidney Kimmel Medical College of Thomas Jefferson University and will provide clinical leadership across Jefferson Health’s regions to support and optimize Enterprise-wide geriatric services. The candidate will identify priorities for and provide clinical leadership on the implementation of initiatives including ambulatory care; hospital consultation; home-based primary, post-acute care and population health initiatives. The candidate will report to the Director of the Division of Geriatric Medicine in the Department of Family and Community Medicine and will be supported administratively by the Associate Administrator for the Division of Geriatric Medicine. The candidate will be appointed as a faculty member of the Sidney Kimmel Medical College at the appropriate rank.
The Enterprise Clinical Lead for Geriatrics will work in close partnership with colleagues across Jefferson including in the academic Division of Geriatric Medicine in the Department of Family and Community Medicine of Sidney Kimmel Medical College at Thomas Jefferson University; Jefferson Primary Care; clinical and operational leaders across Jefferson Health’s three regions, which together encompass 17 hospitals and over 150 ambulatory sites; the Office of Population Health; and Jefferson Health Plan, which is Jefferson’s wholly-owned health plan. The candidate is expected to maintain an active clinical practice in geriatrics.

- Board Certified in Geriatric Medicine/Certificate of Added Qualification (CAQ) in Geriatric Medicine.
- Geriatric Medicine physicians from both Internal Medicine and Family Medicine primary board certification are encouraged to apply.
- At least 5 years’ experience in leadership and development of clinical geriatric programs including experience in operational management, quality, and patient experience
- Demonstrated ability to build and lead interprofessional teams
- Demonstrated ability to effectively partner in a matrixed environment
- Strong understanding of Geriatric Medicine clinical models including considerations of design, business modeling, productivity, quality, experience, and equity, in both traditional and value-based models
- Experience with community collaborations for clinical programming preferred
- Demonstrated commitment to health equity
