Job Opportunity: Geriatrics Provider
Specialty: Geriatrics
Start Date: September 2024
Schedule: Monday – Friday, 8am – 5pm
Support Staff: Nurses, LCSW, Case workers
Coverage Needed per Month: 80 hours per week (1 due to provider leaving and 1 due to growth)
Patients per Day: 1-6/7 (fluctuates depending on the severity of patients' illnesses/medications and time needed)
Patient Population/Age: 55 and older, able to live safely in the community
Department Description: Providing comprehensive care for elderly participants
Key Responsibilities:
- Evaluate and treat participants with episodic illnesses
- Refer participants to medical specialists as indicated
- Judiciously utilize specialty consultants
- Manage all care of participants in the nursing home:
- Provide regular visits
- Perform telephone contacts with nursing home staff as required
- Admit nursing home participants to the hospital when necessary
- Actively participate in family conferences
- Coordinate 24-hour care delivery
- Document participant changes appropriately in the medical record and communicate changes to team members

Practice Setting: Clinic/Private Practice
Requirements:
- Board Certification: BC – will consider non-boarded candidates with reasoning for lapsing
- License Required: NC – due to time frame MUST be NC licensed, valid NC driver’s license, and vehicle
- Certifications Required: Experience caring for the elderly and in good standing and enrolled with Medicare and Medicaid. Need schools attended, NPI, and work experience

- Act within the scope of his or her authority to practice
- Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by CMS before working independently
Credentialing Timeframe: 30-45 days
