Specialty: Geriatrics
Start Date: September 16th or as soon as credentialed
Schedule:
Monday – Friday, 8 AM – 5 PM
Call: 1:3 night call and 24-hour weekend call
Work Details:
Support Staff: Nurses, LCSW, Case Workers
Amount of Coverage Needed per Month: 80 hours per week
Patients per Day: 1-6/7
Patient Population/Age: 55 and older, able to live safely in the community
Key Responsibilities:
Perform comprehensive history and physical on new referrals.
Conduct interval history and physicals on established participants quarterly and as indicated.
Evaluate and treat participants with episodic illnesses.
Refer participants to medical specialists as indicated.
Admit participants to the hospital.
Provide primary care responsibilities for management.
Provide regular updates to the team.
Admit nursing home participants to the hospital when necessary.
Actively participate in family conferences.
Coordinate 24-hour care delivery.
Document participant changes appropriately in the medical record and communicate participant changes to team members.
Participate as a member of the Interdisciplinary Team and take part in participant care planning.
Attend staff meetings.

Requirements:
Board Certification: BC preferred; will consider non-boarded candidates with an explanation for lapsing.
License: Must hold a valid NC license. Due to the time frame, must be NC licensed. Valid NC driver’s license and vehicle required.
Certifications: Experience caring for the elderly, in good standing, and enrolled with Medicare and Medicaid. Need schools attended, NPI, and work experience.
Knowledge, Skills, and Abilities:
Proven clinical leadership.
Ability to excel in a team environment.
Act within the scope of authority to practice.
