This Physician Services Group is a nationally recognized mobile primary care geriatric practice. Their Care Model delivers customized care that is preventive and proactive and includes all members of the care team, patients, and their families. Their patients experience 21% fewer ER visits, 36% less hospitalization, and 41% fewer hospital readmissions compared to patients with similar conditions and complexities.
Help deliver on their promise to tailor primary care to geriatric patients within Assisted Living and Memory Care facilities in and around Brookfield, Wisconsin.
Position Description
- Full-time physicians care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day.
- Collaborate and provide clinical expertise with Nurse Practitioners and Physician Assistants.
- Manage a panel of 200+ patients (approximately 12-16 patients per/day)
- Flexibility in designing and managing your schedule
- Develop and implement care plans appropriate to patients and their desires
- Clinical support from clinical assistants, behavioral care managers, psych consultants, and care coordination through LPNs, RNs, and social workers.
- Access to diagnostics such as labs and x-rays as needed
- Collaborate with community partners within home care, hospice, and other resources as needed
