Submission Requirements:***Must meet ALL submission requirements in order to submit***

• ABFM or ABEM : REQUIRED
• If ABEM certified only NIHSS is required. If FM boarded then ACLS, PALS, ATLS, and BLS
• Active IN License - REQUIRED
• 2 years minimum experience: REQUIRED
• Certifications if ABFM: ATLS, ACLS, BLS, PALS- REQUIRED
• Certifications if ABEM- NIHSS only: REQUIRED
• Must be willing to submit to all three locations: REQUIRED
• IN DEA- REQUIRED
• CSR- REQUIRED
• Clean Malpractice Record: PREFERED

Practice Description:

• Client prefers someone who can work at all three locations.
• Prefer Candidates who can cover 5-7 day stretches at a time.
