**Cardiologist Embedded in Primary Care Team**
Seeking a board-certified or board-eligible Cardiologist to join our primary care team, focusing on coordinated and integrated patient care within a value-based model. This innovative role is designed to improve patient outcomes, enhance access to cardiology, ensure adherence to clinical guidelines, and reduce unnecessary referrals to specialty care.
**Essential Functions**
- Assess and monitor the medical needs of patients and significant others.
- Provide quality, cost-effective medical services to patients, ensuring consistent coverage of caseload.
- Offer input in policy and procedure development related to medical practice and quality patient care.
- Oversee Physician Assistants, Nurse Practitioners, and other delegated care providers.
- Evaluate educational needs of patients, staff, and significant others, implementing or delegating appropriate teaching strategies.
- Maintain documentation consistent with regulatory standards.
- Communicate effectively with other healthcare team members to ensure continuity and coordination of care; advocate for patients and families.
- Participate in process improvement activities through clinical path development and other initiatives.
- Collaborate with Spectrum Health to achieve financial and patient satisfaction goals.
**Qualifications**
- **Education**: Required Bachelor's Degree.
- **Licensure**:
- Physician (MD) or Osteopathic Physician (DO) license in the State of Michigan required upon hire.
- DEA Registration in the State of Michigan required upon hire.
- **Certifications**:
- Basic Life Support (BLS) - required within 120 days, either AHA or ARC.
- Advanced Cardiovascular Life Support (ACLS) - required within 120 days if applicable to specialty or stress test performed.
