Qualifications
• Active CO license - REQUIRED or IMLC (LOQ in hand at time of submission)
• Provider must be available for all shifts or most (minimum 80% of shifts) - REQUIRED
• Clean Malpractice/Background - REQUIRED
• Must include a copy of IMLC at time of name clear
Working Hours
Mon-Fri - 7a-5p + Mon, Tues, Thurs Night call 5p-7a

5p-7a night call to include 2.5 gratis hours
