    • April 21-25
    • June 9-13
    • July 7-11
  • Monday – Friday
  • 7:00am-5:30pm – Cath Lab at the Hospital except for the below days:
  • Every Wednesday 8:00am -5:00pm – In- Office Clinic
  • Every 2nd Friday of the month 8:00am -5:00pm – Telemedicine Clinic
  • Every 4th Friday of the month 8:00am -5:00pm – In-Office Clinic (First Patient at 9:00am) night beeper call on Fridays
  • Patients per day: Clinic 20-22 patients
  • Patient Population/Age: adult
  • Department Description: Cath Lab will be hospital consults 3-4 per day, plus EP cases 4-6 a day depending on case length
  • EMR: Cerner
  • Hospital Bed size: 268
  • Special Procedures: cardiac electrophysiology; specifically, heart rhythm disturbances, Ablations - must show 100 EP cases in the last 2 years in case logs with emphasis on ablations
  • Skills Required: Ablations
  • Practice Setting: Hospital
  • In-Office Clinic
  • BC or BE required
  • License Required: AZ Licensed
  • Willing to License
  • DEA: AZ registered
  • CSR: not required
  • ACLS Required
  • Minimum Experience Required: 1 year
  • Credentialing Timeframe: 30 days
