Requirements:

• Board Certified Otolaryngology – REQUIRED
• Active CO License – REQUIRED or Active IMLC
• Must be able to perform all of the required procedures in job description – REQUIRED TO NOTE AT NAME CLEAR
• Must be able to supervise – REQUIRED TO NOTE
• Active ACLS (AHA) – REQUIRED
• Clean background/no malpractice – REQUIRED
• COVID VACCINATED – REQUIRED
