Requirements:

• Board Certified – REQUIRED if Board Eligible (ABIM ONLY)
• Active WA license or IMLC (LOQ in hand/must upload during name clears) – REQUIRED
• Completed Endocrinology Fellowship – REQUIRED
• Malpractice – Clean Preferred
• Availability at time of name clear – REQUIRED
• ACLS/BLS/DEA – REQUIRED
