• MI license: REQUIRED
• Rates must be submitted at time of name clear – Required will not send to client without rates
• Board certified: Required will not accept BE
• ERCP: Required
• Local Provider – Preferred
• DEA: Required at time of start
• No malpractice – Required no exceptions for quick credentialing
• Covid vaccine – Required
