| SITE INFORMATION: ◊ Pertinent Facility Information: Kaiser Permanente – Fontana Medical Center ◊ Housing/Travel Accommodations (what is allowable & any budgets): Travel & Lodging billed to client if necessary ◊ Credentialing Timeframe: 12 Weeks from time application submitted to client JOB INFORMATION: ◊ Total Number of FTE’s needed? 1.0 FTE ◊ Name Clear Instructions: N/A ◊ Presentation Instructions, Detailed Process, and Timeline: Cover Sheet, CV, and backup documentation regarding malpractice/background – References if we have them on file ◊ Reason for Coverage: Permanent vacancies ◊ Other agencies working and for how long, is anyone presented/pending? N/A ◊ EMR: Health Connect (Kaiser Permanente’s version of EPIC). EPIC Experience highly preferred but at least a good skillset of EMR & Technology required. ◊ Coverage Dates (Specific, long term only): 3 Months/13 Weeks ◊ Shift Schedule Details & Shift Duration: Ideally 2 Shifts during week (10 Hour days M-F) and then Night call and weekend call (Friday-Sunday - 24 hour shifts) ◊ Call Schedule/Responsibilities: Night call on 2 shifts worked and then 24 hour call on weekends. Call back volume extremely low ◊ Type of Patients/Cases: Full scope of pediatric anesthesia cases outside of cardiac & neuro Patients per Day/Volume: ~ ◊ Inpatient or Outpatient: Both ◊ Number of Beds in Unit: N/A ◊ Average Length of Stay: N/A ◊ Miscellaneous Information: ~ REQUIREMENTS: ◊ What is the minimum length of commitment requirement? Minimum number of shifts required per month? 3 months with option to extend ● ● ● Will a client review a Candidate if they do not have full time availability? If so, what is the minimum days per month they would expect per provider? N/A ◊ Procedures/Cases Required: Full scope of pediatric anesthesia, no cardiac or neuro ◊ Certifications/Qualifications Required (ACLS, BLS, MSQA, TEE, etc.): NA ◊ Education Requirements (particular Residencies, particular Fellowships, etc.): NA ◊ Board Certified Required: Certified ● ● ● Specific Specialty Board Required?
