Locums Nurse Practitioner - Mishawaka, IN Located: Midway between Chicago IL & Fort Wayne IN Assignment Dates

  • 6-month assignment to start. Potential for Ongoing. Rate: Negotiable (tell us your rate) Schedule: Negotiable (tell us your preferred schedule)
Coverage Need Full-time preferred (Part-time considered) Scheduled clinic hours with on-call responsibilities: - On-call one week on, one week off - Weekday on-call: 4:30 PM to 8:00 AM - Weekend on-call: 24-hour coverage - Phone-only on-call (no travel required) Position Details Location: Mishawaka, IN EMR: TruChart (transitioning to Epic in March) Geriatric experience required Provides care within a PACE (Program of All-Inclusive Care for the Elderly) model: - Assessments, care planning, treatment, participant education, and team collaboration - Oversight of the clinic and contracted nursing home rounds - Interdisciplinary Team (IDT) participation - Documentation of participant care and medical condition updates Submissions Requirements Active IN license Clean History Board Certified (ANCC or AANP) Up-to-date CV with explanations for gaps >30 days Proof of active IN license and board certification Supporting certifications BLS, CDS, DEA Skill Requirements Geriatric care experience Experience with elderly or frail populations (preferred) Independent judgment and decision-making abilities Strong communication and organizational skills Proficiency in Microsoft Office and EMR systems
