- Our Wound Care APP is responsible for providing comprehensive wound management, using the most advanced and proven methods and materials, while working collaboratively with the hospital wound nursing team. _x000D_
- The Wound Care APP is responsible in creating and maintaining a plan that results in successful wound care outcomes. _x000D_
- The role requires integration health maintenance, disease prevention, physical diagnosis, and treatment of common episodic and chronic problems in primary care with equal emphasis on health teaching and management._x000D_
- Evaluates and manages patients for wound care and ensuring delivered care meets current standards of practice _x000D_
- Coordinates the patient plan of care with the patients PCP or referring provider and the facilitys clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure healing outcome goals _x000D_
- Ensures that education is provided to the facility staff regarding the management of the patients wound(s)._x000D_
- Demonstrates competency in the position of wound care including assessment, advanced modalities, debridement, wound dressings, skin substitutes and total contact casting._x000D_
- Participates in the Quality Improvement Program to optimize quality service and maintain a high level of patient satisfaction. _x000D_
- Serves as a patient advocate, initiating action to provide continuity of care outside the facility setting._x000D_

**Qualifications:**_x000D_
_x000D_
- Board Certified Wound Care Physician, Nurse Practitioner (NP, FNP, ARNP, APRN) or Physician Assistant (PA-C) licensed in Oregon._x000D_
- Active Drug Enforcement Agency (DEA) license_x000D_
- Must have strong interpersonal skills and the ability to communicate effectively with a variety of people through both verbal and written communication lines._x000D_
