We are seeking a full-time nurse practitioner (NP) to join our team working in the Bronx and Westchester county areas. Qualified candidates will have at least one year of previous experience.

You will be providing consultant work at skilled nursing facilities across Bronx and Westchester, New York. Providing follow-up visits following rehab medicine consultations by the physician focused on rehab progress issues including - orthopedic, neurological, amputations, and pain management as a collaborating partner with TeamHealth rehab physician and other facility disciplines. Prior experience with pain management or post acute facilities preferred.

Responsibilities:
  • Drive high-quality, safe, and cost-effective patient care in post-acute care communities
  • Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions
  • Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
Competitive compensation, with an estimated base salary range of $87,000 to $119,000 annually with the opportunity to earn incentives.
