A hospital in Rockland County, NY is currently seeking a licensed Nurse Practitioner (NP) to join their hospice team. In this role, the Nurse Practitioner (NP) will work collaboratively with the Physician team in the Oncology Department to provide palliative care to patients.

Responsibilities:
The Nurse Practitioner (NP) will:
- Manage pain and other symptoms through pharmacological intervention and non-pharmacological intervention
- Provide educational support to patients and families through diagnosis, prognosis, and care options
- Facilitate and advance care planning discussions and help patients express goals of care
- Perform other duties, as needed
Qualifications:
- NYS Nurse Practitioner (NP) license
- Experience in an Oncology, Hospice, and/or Palliative Care role/setting
Desired Skills:
- CHPN
- Working knowledge of Epic EMRs
